Here, you\u2019ll discover a dynamic team of caring and highly-intelligent individuals working to improve the future of healthcare delivery, every day.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t \t\t The Health and Housing Case Management Program is a partnership between Alliance, Duke University, and RHD North Carolina.<\/p> An Alliance study of health care costs at Duke University Hospital found that the vast majority of costs were driven by a fairly small number of people. After identifying those \u201cheavy users,\u201d RHD provided housing and case management \u2014 and quickly reduced hospitalizations by more than 40 percent.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t \t\t Alliance\u2019s community outreach teams host Real World Youth Simulations throughout the year. The Real World provides hands-on practice and learning in the areas of finance, social skills, careers, educational attainment and more. By navigating through the stations, they will gain a new awareness of budgeting and making wise choices.\u201d<\/p> Pamela Story, Cumberland County Social Services<\/strong><\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<\/div>\n\t\t<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t \t\t Bridge housing supports a Housing First<\/a> approach by providing safe and temporary housing for people who are actively seeking permanent supportive housing options and may face additional challenges. This 3-5 month \u201cbridge to independence\u201d program is intended for a more vulnerable population who are exiting jails and other institutions or living in long-term homelessness.<\/p> While in the program, a person is connected to additional services and community resources, receives assistance with housing search and navigation, and coaching to help increase their self-sufficiency whether it be employment, educational attainment, daily living skills, or budgeting.<\/p> The goal of the program is to help a person begin their journey of recovery and community living while putting all necessary wraparound services in place for them to be successful and live the lives they desire.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t \u201cBecause of these amazing partnerships and the efforts of amazing people, over 30 individuals have increased stability in their lives and obtained permanent housing in just the first year and a half of this program!\u201d<\/p> Nicholas Hobbs, RHD North Carolina<\/strong><\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<\/div>\n\t\t<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t Hear from three of our staff members about their experiences with the COVID-19 vaccine, their thoughts on why some are hesitant, and why they ultimately chose to get vaccinated. #FindYourSpot #TakeYourShot #COVID #MentalHealth #PoweredByPeoplepartner and start a new initiative!<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t Erica Asbury is a Member Engagement Specialist at Alliance Health.
